    Excavation Study on the Bupleurum Ancient Literature Based on the Relevance of "Medicinal Property-Efficacy-Toxicity"

    Objective Combing literature about medicinal property, effect, toxicity ofB , to provide Bupleurum chinense,to provide literature accordance and idea for the relevance research of "medicinal property-effect-toxicity". Methods Ancient books, literature in past dynasties about Bupleurum chinense were collated, analyzed and summarized. Results There are different records on the efficacy, indications of Bupleurum chinense in ancient literature were not classified as toxical chemicals in the ancient literature, but in the process of clinical application, some physicians come to realize the toxicity and side effects of Bupleurum chinense. Conclusion Literature study shows that there are certain toxicity and side effects of Bupleurum chinense , consistency with the records about Bupleurum chinense "robbing hepatic yin" and modern clinical adverse reactions. Some physicians believe that is the first drug to treat "liver qi stagnation". At present the researches of the medicinal, effect, material basis and toxicity in are isolated, so how to balance the relationship between "shuganjieyu" and" Bupleurum chinenserobbing hepatic yin", to observe and find the toxicity starting from the effect of Bupleurum chinense, and using modern language to clarify the intrinsic nature relationship between effect and toxicity of becomea problem that should be solved urgently to be more effective and safe application of Bupleurum chinense .

    Effect of Platycodon Grandiflorum(Jaoq.)A.DC on the Micronuclei Frequencies Induced by Mitomycin in the Bone Marrow Cells of Mice

    Objective To observe the inhibitory effects of Platycodon grandiflorum(Jaoq.) A.DC on micronuclei frequencies induced by MMC in the bone marrow cells of mice. Methods Fifty mice were divided into groups: negative control (N·S), positive control (MMC 0.4 mg·kg-1), Platycodon Grandiflorum(Jaoq.)A.DC mutagenesis groups (1.0、2.0、4.0 and 8.0g·kg-1, which were matched5、10、20、40 clinical commonly used dosage ofa person) Platycodon Grandiflorum(Jaoq.)A.DC antimutagenesis groups ( MMC 30mg·kg -1 + each Platycodon Grandiflorum(Jaoq.)A.DC dosage Platycodon Grandiflorum(Jaoq.)A.DC groups), 5 mice per groups. MMC are given by drug abdominal cavity Injection, other drug are given by perfusing stomach. Platycodon Grandiflorum(Jaoq.)A.DCand MMC are given simultaneously in antimutagenesis groups, oncea day for 5 days successively. At the 5th weekend after the first administration, the mice are killed by cervical vertebra dislocation whose marrow are drawed and sheets are produced in our laboratory method. Results the micro-nuclei frequencies was no significant difference between tested groups and control group(P<0.05), but the frequency of micronucle induced by MMC was reduced by intragastrial perfusion of Platycodon Grandiflorum(Jaoq.)A.DC (4.0g·kg-1, 8.0g·kg-1) and MMC at the same time(约0.01). Conclusion Platycodon Grandiflorum(Jaoq.)A.DC can not induce micronulei formation in the mouse bone marrow cells, but showsa protective effect on genetic materials in mice.

    Comparative Study on Acute Toxicity of Different Components of Rhizoma Pinelliae in Mice

    Objective Mice's acute toxicity of different components of Rhizoma Pinelliae was compared, to make an observation and evaluation of acute toxicity of Rhizoma Pinelliae in order to providea basis for safe and reasonable medication in clinic. Methods Preparing the all components,water extraction components and alcohol extraction components,then ig. all-components,water extract and alcohol extract of Rhizoma Pinelliae to mice and the MLD, MLD, MTD were tested respectively by the classic Methods of acute toxicity. Results The MLD of all-components is 34.8g/kg;the MLD of water extract is 300.0g/kg and the MTD of alcohol extract is 99.2g/kg,which is respectively equal to 270.7 times,2 333.3 times and 771.6 times of 70kg people's daily dried medicinal herb expenses. Conclusion Rhizoma Pinelliae has certain toxicity,which is consistent with the pharmacopoeia,references and clinical application.The mice's acute toxicity of different components of Rhizoma Pinelliae is alcohol extraction components >water extraction components and all-components, while the last two components are atoxic on the whole.

Conclusion The producing area has certain influence on the content of toxical substances and acute toxicity of Rhizoma Pinelliae, and the toxicity of Rhizoma Pinelliae has some relevance with the content of total organic acid and total alkaloid.

    Objective To compare the content of toxical substances and the acute toxicity of Shandong Pinelliae's different extractions, and discuss the extraction's influences on the content of toxical substances and acute toxicity. Methods UV method, HPLC method, acid -base titration and the classical method of acute toxicity were used to determine the content of toxical substances and compare the acute toxicity of Pinelliae and its extraction. Results The content of total organic acid,total alkaloid and vernine is: Pinelliae> extraction;the content of amber acid and ephedrine is: extraction>Pinelliae .The MLD of Pinelliaeis 301.2g/kg,and its extraction is 374.4g/kg, which is respectively equal to2 342.7 times, 2 912.0 times of 70kg people's daily dried medicinal herb expenses. Conclusion Extraction has certain influence on the content of toxical substances and acute toxicity of Pinelliae, and the toxicity of Pinelliaeis higher than its extraction.

    Objective To compare the content of toxical substances and the acute toxicity of Rhizoma Pinelliae in different processing, and evaluate the processing's influences on the content of toxical substances and acute toxicity. Methods UV method, HPLC method, acid -base titration and the classical method of acute toxicity were used to determine the content of toxical substances and compare the acute toxicity of Rhizoma Pinelliae in different processing. Results The content of total organic acid is: 75% alcohol-infiltration>75% alcohol-extraction>30%alcohol-extraction> 30% alcohol -infiltration >water -infiltration抑water -extraction; the content of total alkaloid is: 75% alcohol -infiltration> 75% alcohol -extraction >30% alcohol -infiltration >30% alcohol -extraction >water -infiltration抑water -extraction. MTD value can betested in 75% alcohol -infiltration and 75% alcohol -extraction groups, while MLD value can be tested in other groups. The comparation of acute toxicity is: 75% alcohol-infiltration>75% alcohol-extraction>30% alcohol-infiltration>30% alcohol-extraction >water-infiltration >water -extraction. Conclusion Extraction has certain influence on the content of toxical substances and acute toxicity of , and the toxicity of Rhizoma Pinelliae has some relevance with the content of total organic acid and total alkaloid.

    Study on the Adverse Drug Reaction Monitoring from Reporting Time

    Objective To study the adverse drug reaction(ADR) reporting time for improving the monitoring of ADR. Methods The occurrence time and reporting time of new and serious ADR were analyzed. Results The new and serious ADR occurs at the same day accounted for 60.03% . The ratio of reporting time according to national laws accounted for 26.09%. The death case accounted for 21.95%. Conclusion s Enhancing the train and Propaganda of ADR knowledge and the strengthening reports consciousness will improve the timeliness of ADR.

    The Effect of Eliminated Known Adverse Drug Reaction Database of Spontaneous Reporting System for Signal Detection

    Objective To study the quantitative effect and optimized strategy of eliminate known ADR database of spontaneous reporting system for signal detection. Methods Analyze 60 996 ADR reports of 2007 ~2009 from Shanghai Center for adverse Drug Reaction Monitoring by MHRA. Results The detected single number of eliminate known ADR reports of drug of interest all other reactions of 2007 ~2009 could added 164、183、200 and detected rate was raised 48.9% compare with not eliminate known ADR. And found new important ADR signals 17、11、14 respectively. Eliminate known ADR of reactions of interest-all other drugs and eliminate all known ADR simultaneously could added number of ADR signal, but they could made important signals lost. Such as acanthopanax injection bring about liver damage have not been reported up to now in other literatures. Conclusion The strategy of eliminate known ADR database of drug of interest all other reactions can improve the efficiency of ADR signal detection and it is easy to operate.

    Analysis of Significance of Pharmacovigilance in Hospital

    Objective By analyzing the characteristicsand regulars of adverse drug reaction/event(ADR/ADE) occurred in one hospital, to provide reference for clinical rational drug use. Methods To adopt the reviewing analytical method to count up and analyze 237 cases ADR/ADE report,these ADR/ADE reports were received from Jan. 2006 to dec.2008 by one hospital. Results Out of the 237 cases ADR/ADE reports,58 showed ADR among the over 60 year old group, amount to 24.47%. Leading the list were antibiotics(56.92%).The clinical expression of ADR is showed mostly skin and appendix harms, amount to 39.18% .The suspected ADR cases induced by intravenous route were most common,amount to 75.10%. Conclusion ADR monitoring is an important work for medical institutions, and pharmacovigilance is conducive to patients' medication safety.

    Objective To explore quality analysis of adverse events of medical device(MDAEs) Reports with characteristic of MDAEs Reports and quality standard of adverse drug reactions Reports. Methods Quality scoring table is prepared with the Weighted Coefficient of each item in Reports by using literature research, questionnaire and small group discussion. MDAEs Reports of a random 5% of MDAEs Reports database from Jan. to Apr. 2010 in center for drug deevaluation of SFDA were analysed with the scoring table, that is to validate feasibility and merit of Quality scoring table. Results Quality scoring method of MDAEs Reports is established. Conclusion The quality scoring method is scientific, effective and easy to be operated. The Results are accurate. It is propitious to improve quality of MDAEs Reports and to furnish true information for risk management of supervisory board.

    Review of Safety and Effectiveness of Carbazochrome Tablets and Injection

    Objective To evaluate the safety and effectiveness of Carbazochrome tablet and injection. Methods Clinical trial and adverse drug reaction report articles retrieved in Chinese Science and Technology Journals Database(1989~2009) and Chinese Journals Full text Database(1994~2009) were evaluated. Results 13 clinical trials, 858 patients were induced in 13 articles for carbazochrome tablet and injection, but none is randomized, controlled trial. Conclusion Study design for clinical trial of carbazochrome should be improved. The formula of carbazochrome injection is not optimized, and the ratio of risk to effectiveness of the injection is unacceptable. There has been other substitute and carbazochrome injection should be terminated.

    Analysis on 829 Serious Adverse Drug Reaction/ Event Reports

    Objective To understand the serious adverse drug reactions/events(ADR/ADE) occurred in the characteristics and rules for clinical safe, effective and rational use of available information. Methods of Jinan City in 2009 collected 829 cases of serious ADR/ADE reports were retrospectively analyzed the data. Results 829 cases of serious ADR/ADE involving 273 drugs, which occupies the first anti-microbial drugs(accounting for 44.75%); involving 75 varieties of national essential drugs( accounting for 27 .47 % ) , in which serious ADR/ ADE cases , 270 cases ( accounting for 32.57%); route of administration by intravenous infusion of the largest proportion (accounting for 78.04% ); serious ADR/ADE main clinical manifestations of anaphylactic shock(accounting for 8.56%), respiratory, systemic and central nervous system damage. Conclusion strengthening serious ADR/ADE monitoring, to ensure clinical safety, effective, rational use of drug.

    Analysis of 631 Reports of ADR/ADE Caused by Xuesaitong Injection

    Objective To study the general pattern and characteristics of adverse drug reactions induced by Xuesaitong injection to provide references for clinical rational use of drug. Methods 631 ADR/ADE reports of Xuesaitong injection collected by Henan center for ADR monitoring from 2004 to 2009 were analyzed using retrospective study method. Results 259 cases of patients over the age of 60 accounted for 41.05% ;233 cases of patients with ADR/ADE occurred within 30 minutes accounted for 36.93%; 186 cases of drug overdose accounted for 29.48%;125 cases of combination therapy accounted for 19.81%; ADR/ADE mainly were skin allergies. Conclusion ADR monitoring should be strengthened to improve the rational use of drug and reduce the occurrence of ADR/ADE.

    Analysis of 108 Adverse Drug Reaction Cases of Fluoroquinolones

    Objective To summarize the adverse drug reaction(ADR) of fluoroquinolones and provide reference for safe and reasonable drug use. Methods 108 ADR cases of fluoroquinolones were retrospective analyzed. Results Adverse reaction was found in all organs and it happens easily in combined medicine, and the incidence rate in digestive system, skin and adnexa, cardiovascular system and central nervous system were 19.44%, 18.52%, 15.74% and 13.89%. Conclusion ADR of fluoroquinolons was found highly in clinical system and organ, and it must be payed attention to ADR of fluoroquinolones in clinical use.
