    Effects of EGFL7-blocking Monoclone Antibody Inhibiting Proliferation and Invasion of Mouse Adrenocorticotropic Hormone Pituitary Adenoma AtT-20 Cells in vitro

    Objective To study the effects of EGFL7-blocking monoclone antibody inhibiting the proliferation and invasion of adrenocorticotropic hormone (ACTH) pituitary adenoma AtT-20 cells in vitro. Methods After treated with different concentrations of EGFL7-blocking monoclone antibody for 24, 48, 72 h, the proliferation of AtT-20 cells was determined by MTS. Simultaneously, ACTH secretion of AtT-20 cells by ELISA and the mRNA expression of invasion related genes such as E-cadherin, snail, vimentin were detected by Real-time PCR. Results EGFL7-blocking monoclone antibody could effectively inhibit the proliferation and ACTH secretion of AtT-20 cells in a dose-dependent and time-dependent manner. Also, EGFL7-blocking monoclone antibody significant down-regulated the mRNA level of E-cadherin、snail, vimentin in AtT-20 cells. Conclusion EGFL7-blocking monoclone antibody could inhibit mouse pituitary adenoma AtT-20 cells proliferation and invasion.

    Effects of Phellinus igniarius on MPP+-induced Apoptosis in SH-SY5Y Cell Line

    Objective To evaluate the inhibitory effects of water extract of Phellinus igniarius on MPP+-induced apoptosis in SH-SY5Y cell line. Methods The relative apoptosis rate was analyzed by PI staining. Morphological changes of nucleus were observed by DAPI staining. Changes of mitochondrial membrane potential were detected by JC-1 staining. ROS production was measured by C-DCDHF-DA staining. Results The relative apoptosis rate of SH-SY5Y cells increased obviously after MPP+ induction, the mitochondrial membrane potential decreased remarkably, and ROS production increased significantly. Compared with the MPP+ control group, the apoptosis rates were significantly decreased by Phellinus igniarius at doses of 20 and 10 μg·mL-1 respectively. The morphological changes of cell nuclei after apoptosis were reversed by Phellinus igniarius. Phellinus igniarius treatment (20, 10, 5, 2.5 μg·mL-1) dramatically increased relative mitochondrial membrane potential compared with MPP+ control respectively. Compared with the MPP+ control, Phellinus igniarius treatment (20, 10, 2.5 μg·mL-1) significantly decreased relative ROS formation respectively. Conclusion The water extract of Phellinus igniarius showed significant inhibitory effect on SH-SY5Y cells apoptosis induced by MPP+.

    Efficacy of Aprepitant in Prevention of Vomiting During Multiple Cycles of Cisplatin-based Chemotherapy in Lung Cancer

    Objective To analyze the efficacy of aprepitant in the prevention of vomiting during multiple cycles of cisplatin-based chemotherapy in lung cancer. Methods 95 cases of patients with lung cancers who received four or six cycles of cisplatin-based chemotherapy were selected retrospectively. All the patients received antiemetic regimen including aprepitant: aprepitant (125 mg po D1, 80 mg po D2~D3), ondansetron (8 mg iv bid D1~D3) and dexamethasone (5 mg iv D1-D3). The complete response rate of vomitting, nausea-free rate and adverse effect were observed. Results 24 and 73 patients received 4 and 6 cycles of cisplatin-based chemotherapy respectively. Complete response rates ranged from 71.2% to 78.9% with no significant decline during continuous cycles of chemotherapy. 61.1% to 68.4% of patients were nausea-free during chemotherapy. The reported side effects were mild fatigue, constipation, hiccup, diarrhea and insomnia. Conclusion The combination of aprepitant, ondansetron and dexamethasone enhanced the antiemetic protection, and its efficacy was also sustained for up to six cycles of cisplatin-based chemotherapy in lung cancer patients.

    Lifecycle Management of American Drug Labeling and Its Implication

    Objective To put forward suggestions for improving drug labeling regulatory system and establishing a dynamic management system for drug labeling. Methods By studying American laws, regulations and guidelines of drug labeling, its scientific points were analyzed. Results and Conclusion It was suggested that we should learn the management pattern of America, improve the legal system of drug labeling in China, strengthen the classification management of labeling, set different labeling types based on different readers, improve the post-market drug labeling revision procedure. Generic drugs labeling should be consistent with new drugs labeling in principle. Penalties for drug labeling violation should be enhanced in order to promote the initiative of drug companies to revise labeling, accelerate the update of drug labeling, and finally form a dynamic adjustment mechanism throughout the drug lifecycle.

    Overview of Incentives for Rare Disease Drugs Research and Development in the United States and Enlightenment for China

    Objective To analyze the general situation of incentive policies on rare disease drugs research and development in the United States, so as to provide reference for the establishment of policies on rare disease drugs research and development in China. Methods By comparing the differences between China and the United States in terms of rare diseases legislation, official definition of rare diseases, and incentive measures on rare disease drugs research and development, the inadequacies of incentive policies on rare disease drugs research and development in China were pointed out, and specific suggestions were putted forward. Results The United States was the first in the world to promote development of rare disease drugs through legislation, and has a clear official definition of rare diseases. In the United States, incentive policies and measures, such as funding research and development projects and speeding up the examination and approval process, have greatly increased the number of rare diseases drug and improved medication status of rare diseases patients. Although China has carried on the exploration to policies on rare disease drugs research and development, but there is no official definition of rare disease. There is lack of incentives to develop rare disease drugs, accessibility of rare disease drugs needs to be further improved. Conclusion China should learn from foreign advanced experience, establish management incentive policies on rare disease drugs research and development, so as to protect legal rights and interests of rare diseases patients.

    Study on Data Masking Effect Reduction Method for Adverse Drug Reaction Based on Hybrid Model

    Objective To propose a data masking effect reduction method based on hybrid model to solve the problem of masking in adverse drug reactions (ADRs) signal detection by traditional disproportionality measurement. Methods The model was mixed together with Lasso Logistic Regression(LLR), report removal and IC method. The first round of data removal was carried out by Lasso Logistic Regression method and the second round was to remove the reports which frequency is less than or equal to 4. Then, IC algorithm was used for signal detection. Finally, traditional Chinese medicine data of ADR monitoring reports of China in 2010 was used to conduct experiments, and the Results were tested and verified based on known adverse reaction database and Information Bulletin of National ADR Center. Results The experimental Results showed that this model had better performances in recall rate, precision and F index by comparison with each single method. Conclusion The hybrid model not only improves the accuracy of the ADR signal detection, but also has the ability to detect more true positive signals at early stage. This research may provide a more reliable method for ADR signal detection.

    Objective To provide reference for rational clinical application of Polygoni Cuspidati Rhizoma. Methods By combing clinical application and safety information in ancient traditional Chinese medical books and modern literatures, the similarities and differences of Polygoni Cuspidati Rhizoma clinical application and safety in ancient and modern were compared. Results The clinical application of Polygoni Cuspidati Rhizoma reported in the ancient books and modern literature were basically same. However, compared with ancient books, there were more adverse reactions reported in modern literature. Its mainly symptoms were gastrointestinal reactions which were all caused by compound preparation. Conclusion The application of Polygoni Cuspidati Rhizoma in compound preparation may increase the possobility of adverse reactions. We need to pay more attention on the compatibility of Polygoni Cuspidati Rhizoma as well as dosage during the clinical utilization.

    Literature Analysis of 95 Cases of Adverse Drug Reactions Induced by Vancomycin

    Objective To explore the causes and manifestations of adverse reactions by analyzing the cases of vancomycin induced adverse reactions, so as to provide references for clinical treatment. Methods 'Vancomycin', 'adverse reaction' and 'caused by' were used as key words to retrieve articles in CNKI, VIP and WANFANG database. 77 literatures which met the inclusion and exclusion criteria were extracted. 82 cases of ADR in literatures were extracted, analyzed and summarized. Results The adverse reactions of vancomycin were mainly manifested as red man syndrome, acute renal impairment, fever, multiple blood cell loss, etc. Conclusion It is necessary to standardize the clinical medication and strengthen the patient's medication monitoring, and to monitor the blood concentration of patients when necessary.

    Literature Analysis of 151 Cases of Adverse Drug Reactions Induced by Metoclopramide

    Objective To provide technical support for safe and rational use of drugs in clinic by investigating the characteristics and mechanism of adverse reactions induced by metoclopramide. Methods 'Metoclopramide', 'adverse reaction' and 'to' were used as key words to search and retrieve articles in the Wanfang database, CNKI database and VIP Chinese full-text database. 34 references which met the inclusion criteria were extracted and analyzed. Results 151 cases were extracted. Adverse reactions mainly occurred in children. Majority of indications were vomiting. ADRs occurred more frequently within 1h. Systems-organs mainly involved in adverse reactions were systemic disorders, skin and its accessory disorders and gastrointestinal disorders etc. Extrapyramidal reactions were the most commonly reported adverse reactions. It can cause serious adverse reactions such as anaphylactic shock and even death. Conclusion It is important to pay attention to the adverse reactions caused by metoclopramide, pay attention to extrapy ramidal reactions and strengthen clinical rational use of drug, so as to ensure the safe drug use in clinic.

    Review and Analysis of Unreasonable Use of Propatamol after Operation in Our Hospital

    Objective To evaluate the clinical application of propacetamol in order to improve the reasonable application level. Methods From August to November in 2017, all case histories which used propacetamol after operation (532 copies) were selected from the HIS system in our hospital and evaluated according to the relevant guidelines and instructions of propacetamol. The evaluation contents included safety, rationality and so on, and the evaluation Results were summarized and analyzed. Results There were 178 unreasonable case histories of propacetamol, and the unreasonable rate was 33.46%. The main problems were too slow infusion speed, too long treatment course, unreasonable drug combination, over-maximum daily dose, over-age limit and so on. Conclusion The unreasonable application of propacetamol in our hospital is common, and its reasonable application level needs to be improved.
