    Study on Screening Natural Flavonoids Inhibiting GH3 Cells by High-content Screening Technology

    Objective To screen the natural flavonoids inhibiting growth hormone adenoma GH3 cells using the high-content screening(HCS) technology, and explore their mechanism of action.Methods After treated with 13 natural flavonoids for 24 and 48 h, the growth morphology and proliferation activity of GH3 cells were determined by HCS. Simultaneously, the apoptosis ratios of GH3 induced by natural flavonoids were detected by Annexin V-FITC/PI. Apoptosis of GH3 cells induced by compounds were detected by flow cytometry.Results Allylisothiocyante screened from 13 natural flavonoids could effectively inhibit the proliferation of GH3 cells in a time-dependent manner and induced apoptosis of GH3 cells dose-dependently.Conclusion High-content screening technology is a powerful tool and reliable method for screening drugs via tracking the in vitro antitumor function of drugs in the morphological changes and proliferation of GH3 cells. The compound allylisothiocyante screening by HCS inhibited the proliferation of GH3 cells through inducing cell apoptosis.

    Auxiliary Therapeutic Effects of Emodin on Diabetic Nephropathy

    Objective To investigate the auxiliary therapeutic effects and mechanism of emodin on diabetic nephropathy(DN).Methods DN rats were induced with GK rats fed with high fat diet. Forty DN rats were divided into 5 groups: model group, repaglinide (0.7 mg·kg-1) group, emodin (20, 40 and 80 mg·kg-1) plus repaglinide (0.7 mg·kg-1) group according to the level of blood glucose. Each group consists of 8 rats. Animals were administered orally twice daily for continuous sixty days. Hemorheology index, blood glucose(GLU), blood fat(TC、TG), inflammatory cytokines(IL-6、TNF-α) and renal function index(BUN、CR、mAlb) were determined after the last intragastric administration, and histopathological changes of kidney were observed by HE staining.Results Compared with repaglinide group, the level of TC, TG, BUN, CR, IL-6, TNF-α, mAlb and hemorheology index were decreased, at the same time pathological changes of kidney were alleviated significantly in emodin (40 and 80 mg·kg-1) plus repaglinide (0.7 mg·kg-1) groups.Conclusion Emodin (40 and 80 mg·kg-1) has a remarkable renal protective effects on kidney of DN rats.

    Analysis of Adverse Reactions on the Elevated Triglyceride Levels Caused by Tretinoin Tablets in Treatment of Patients with Psoriasis Vulgaris

    Objective To investigate the effect of tretinoin tablets in treatment of patients with psoriasis vulgaris on triglycerides.Methods Forty-nine patients with psoriasis vulgaris were enrolled from May 2014 to April 2016 in our hospital and randomly assigned to the control group(n=24) and observation group(n=25). The triglyceride levels of two groups were detected before treatment. The control group received anti-inflammatory or support therapy and the observation group received anti-inflammatory or support therapy plus taking tretinoin tablet 10 mg each time by three times a day. The triglyceride levels of two groups were detected after treatment. The paired t1 test were used for comparison the triglyceride levels before and after treatment, and independent sample t2 test were used for comparison between the triglyceride levels of two groups.Results The average time of therapy of control group was (9.05±1.61) days. The triglyceride levels of control group before and after treatment were (1.09±0.64) mmol·L-1, (1.25±0.47) mmol·L-1 respectively. The difference of them had no statistical significance (t1=0.623,P >0.623). The average time of therapy of observation group was(8.47±1.64) days. The triglyceride levels of observation group before and after treatment were (1.27±0.45) mmol·L-1, (2.39±0.85) mmol·L-1 respectively. The difference of them was statistically significant(t1=5.488, P <0.01). But the difference of the treatment time of two groups was not statistically significant (t2=1.095,P >0.05). The difference of the triglyceride levels of two groups before treatment also had no statistical difference (t2=1.035, P >1.035), but the triglyceride levels of two groups after treatment were statistical significant (t2=5.039, P <0.01).Conclusion Tretinoin tablets in treatment of patients with psoriasis vulgaris could raise the triglyceride levels in a short period of time. Early detecting the blood lipid changes in patients after treatment could effectively reduce the risk of cardiovascular disease in patients.

    Analysis on Global Research Trends of Pharmacovigilance in Recent Five Years and Its Implications for China

    Objective To analyze the global research trends of pharmacovigilance by using an intelligence analysis method, so as to explore the implications for pharmacovigilance research in China.Methods Data derived from core set of Web of Science. Bibliometrics, social network analysis and co-word clustering analysis were used to analyze institutions, foundations, core journals and research hot spots in terms of global pharmacovigilance research trends.Results Staged growth and main institutions are universities, hospitals, academic institutions and pharmaceutical enterprises, and collaborations between institutions were tight. The funding sources were diverse, including pharmaceutical enterprises, institutions, academic institutions and key projects. Fourteen core journals, such as Drug Safety, and the annual meeting of the international society of pharmacovigilance were the main communication carriers and approaches. There were seven hot spots which focused on three directions. a) The pharmacovigilance signal management and pharmacovigilance database data mining based on big data and artificial intelligence algorithm. b) The safety of vaccines and special populations, such as children and the pregnant, and pharmacovigilance studies about biological agents and biosimilars. c) The application of social media in the monitoring of adverse drug events. All above are the global research trends of pharmacovigilance.Conclusion a) To increase the intensity of government policies'guidance; b) To encourage inter-agency collaborations and seek international collaborations; c) to strengthen R&D investment by engaging diverse funds; d) To improve the pharmacovigilance data system and explore research activities with characteristics of TCM. These will promote research studies on pharmacovigilance in China.

    Objective To improve and guarantee the quality of traditional Chinese medicine in medical institutions, protect and support the pharmaceutical preparations of medical institutions, and promote the development of traditional Chinese medicine in China.Methods Through establishing a market leverage price system that the quality classification system of Chinese herbal medicine and herbal decoction pieces matches with the market price lever of traditional Chinese medicine in medical institutions, strengthening the detectability construction of medical institutions' drug quality control rooms, establishing the regional center of preparation room, setting up traditional Chinese medicine quality risk detection platform for medical institutions, speeding up research on the standardization of traditional Chinese medicine preparations in medical institutions and improving the standardization research on Chinese medicine preparation method to promote the construction of quality assurance.Results We have formed a mechanism for improving and guaranteeing the quality of the traditional Chinese medicine preparations.Conclusion Traditional Chinese medicine preparations in medical institutions is an important part of Chinese medicine clinic, in view of the quality of traditional Chinese medicine preparations in medical institutions, take effective measures to improve the quality, ensure the safety and effectiveness of traditional Chinese medicine preparations for clinical use.

    Objective To explore the application of metabolomics in the evaluation of traditional Chinese medicine (TCM) toxicity.Methods The reports on toxicity mechanism and detoxification mechanism of TCM have been collated, analyzed and summarized.Results Metabolomics has been involved in the study of the toxicity mechanism of TCM, including liver, kidney, lung, heart, nervous system, reproductive system, gastrointestinal tract and other target organs. And processing methods and the compatibility of TCM were found to reduce the target organ damage.Conclusion From TCM toxicity mechanism research to detoxification mechanism research, the traditional evaluation based on experience is transformed into a modern evaluation based on scientific methods and standards. The development of metabolomics would make the research of TCM more scientific, normative and standard.

    Clinical Pharmacy Service Mode and Its Effect on Patients with Liver Dysfunction

    Objective To observe the effects of clinical pharmacy service mode in patients with liver dysfunction, and to explore the pharmaceutical service method.Methods 315 cases of patients with liver injury in our hospital from June 2016 to May 2017 were included for this study. According to the time of admission,158 patients from June to November in 2016 were assigned to the control group, 157 patients from December 2016 to May 2017 were assigned to the observation group. Patients in the observation group received standardized clinical pharmacy service according to Child-Turcotte-Pugh (CTP) score, and the control group only received conventional pharmacy services. The indicators of pharmacodynamics, economics, rational use of drugs and pharmaceutical service effects were compared between the two groups, and the experience of clinical pharmacy service mode was summarized.Results The total efficacy of the observation group was 98.09%, which was higher than 89.87% of the control group. The hospitalization duration, the total hospitalization expenses, the cost of protecting the liver drug, the total cost of medication and the total cost of medication/total hospitalization expenses ratio in the observation group were lower than those in the control group, and the difference was statistically significant (P <0.05) .The incidence rate of ADR of control group was 13.92%, which was higher than 6.37% of the observation group. The pharmacy service awareness, compliance and satisfaction of observation group were higher than the control group, the difference was statistically significant (P <0.05).Conclusion For patients with liver function damage, the standardization of clinical pharmacy service mode can improve the treatment effects, reduce the cost of treatment, ensure the rational use of drugs and improve the effects of pharmaceutical care, which is worthy of optimization and promotion.

    Review on Cases and Literature of Flurbiprofen (Axetil)-induced Liver Damage

    Objective To evaluate the association between flurbiprofen axetil and liver damage in clinical use.Methods Taking flurbiprofen (axetil) as subject terms, and adverse drug reactions, hepatotoxicity, hepatic damage, transaminases and hyperbilirubin as search term, the authors collected literatures from CNKI, WANFANG Medical Database, VIP Database, Pubmed, Medline and Cochrane CENTRAL Database published at home and abroad from January 1980 to November 2017. The literatures were read and screened for the ADR case reports and drug safety research reports into the study. Data record sheets was designed and the basic information, method feature and intervention were picked up and recorded. Using descriptive research method, the authors evaluated the probability, extent and type of liver damage caused by flurbiprofen (axetil).Results There were 3 randomized trials studies, 1 prescription statistic study, 1 case statistic study and 3 individual case data studies in 8 papers screened. By analyzing 1 210 patients in 3 randomized trials studies, the authors found that about 1% of patients have abnormal liver function. Then they concluded that flurbiprofen (axetil)-induced liver damage was not an uncommon adverse effect. 6 literatures recorded abnormal indicators of liver function without the changes of the bilirubin index. 5 of them indicated abnormal transaminase index and 2 of them indicated abnormal alkaline phosphatase index. The degree of liver damage is judged to be mild and abnormal transaminase to be the main type of liver injury.Conclusion The authors think that flurbiprofen (axetil)-induced liver damage is a common adverse effect and the degree of injury is mainly mild and abnormal transaminase is judged as the main type of liver injury. Considering the limited number and poor quality of current literatures, the relationship between flurbiprofen (axetil) and liver damage needs further study with more clinical statistical analysis in the authors' hospitals.

    Analysis of 413 Cases of Adverse Drug Reactions Induced by Fluorescein Sodium Injection

    Objective To investigate the characteristics and mechanisms of adverse drug reactions (ADRs) induced by fluorescein sodium injection, so as to provide technical supports for safe and rational use of drug in clinic.Methods A total of 413 cases of fluorescein sodium injection-caused ADRs were reported in Zhejiang province from January 2010 to June 2017 and analyzed by Excel software.Results The male cases were slightly more than the females. The adverse reactions were in a rapid reactive type. System organ classes(SOCs) mainly involved in adverse reactions were gastrointestinal disorders, accounted for 58.58%, followed by skin and appendages disorders, application site disorders. Serious ADRs mainly involved the general disorders. Allergy test can not completely predict ADRs.Conclusion The adverse reactions caused by fluorescein sodium injection should be paid attention, especially the serious ADRs such as anaphylactic shock and so on. Medical staff should do well in the prevention, monitoring and treatment of ADRs.

    Analysis and Pharmacovigilance Thinking on Chinese Patent Medicine Containing Psoraleae Fructus

    Objective To analyze the characteristics of hepatic injury caused by the Chinese patent medicine which contained Psoraleae fructus by collecting relevant reports systematically, so as to provide pharmacovigilance thinking for the pharmacovigilance and rational use of them.Methods The article reports about hepatic injury caused by the Chinese patent medicine contained Psoraleae fructus in the National Healthcare Directory were screened and searched. The untoward effects of this Chinese herbal piece were analyzed by using retrospective study and statistical methods and combine with the ancient recordation.Results A total of 42 hepatic injury cases in 26 literatures were collected, which involved 11 kinds of Chinese patent medicine contained Psoraleae fructus. According to the analysis, this kind of adverse reactions mainly concentrated in the middle-aged and old people with the characteristic of normal dosage and long-term treatment as well as favourable prognosis.Conclusion To strengthen the pharmacovigilance of Chinese patent medicine contained Psoraleae fructus is of great significance in improving the safety and reasonable application of them.

    Analysis of the Utilization of Oxygen Atomization Inhalation Given to Inpatients of Our Hospital

    Objective To provide references for clinical rational use of medicines by analyzing the utilization of oxygen atomization inhalation for the inpatient in our hospital.Methods From the clinical pharmacy workstation, sift the inpatients who were given with oxygen atomization inhalation, then retrospectively analyze the utilization and evaluate the relevant information.Results The major departments using oxygen atomization inhalation were the respiratory medicine, cardiovascular medicine, general surgery, osteology and cardio-thoracic surgery, the proportions were 15.25%, 11.86, 10.89% and 10.17%. Patients under 10 years old (22.03%) and over 51 years old(29.06%) have higher proportions than the others. The medicines used in the largest amount were dexamethasone sodium phosphate injection, 0.9% sodium chloride injection, budesonide suspension for inhalation, ambroxol hydrochloride injection, chymotrypsin for injection. The single average dose was 20.81mL. The proportions of two medicines combination and three medicines combination were 35.53% and 41.65% respectively. The most commonly used medicine combination was terbutaline sulphate solution for nebulization and budesonide suspension for inhalation.Conclusion The oxygen atomization inhalation for patients is basically rational in our hospital, but unlabeled route of administration, the overdose use of medicine in a single time and the irrationally combined use of different medicines existed. Combine the medication guidance of clinical pharmacists and closely cooperate with clinicians to promote the safe, effective and rational use of the oxygen atomization inhalation.

    Objective To explore how clinical pharmacists to use antithrombotic drugs for patients with pulmonary embolism after percutaneous coronary intervention operation to carry out pharmaceutical care.Methods By studying the antithrombotic drugs used in the patient, the possible causes of gastrointestinal hemorrhage were analyzed.Results The application of anticoagulant and antiplatelet drugs in the quadruple is the main cause of the symptoms of gastrointestinal hemorrhage in this patient.Conclusion The application of anticoagulants should be based on the risk of bleeding and the types of anticoagulant drugs for patients with heparin and wafarin indication when aspirin and clopidogrel are applied after percutaneous coronary intervention.
