Abstract: Objective To discuss the approaches for clinical pharmacists to carry out pharmaceutical care. Methods Combining the disease characteristics of the patient and medication history, clinical pharmacist provided the patient with individualized pharmaceutical care. Results The treatment-related problems in time were identified and resolved by clinical pharmacists, and the pharmacotherapy was effective and rational. Conclusion Clinical pharmacists can provide pharmaceutical services to ensure the safety and effectiveness of drug use.
