Abstract: Objective To investigate the adverse drug reaction of euglycemic diabetic ketoacidosis (euDKA) associated with dapagliflozin used in high-altitude areas in order to provide a reference for safe and rational drug use. Methods A case of euDKA in a 26 year old type 2 diabetes patient with long term high altitude residence and after prolonged use of dapagliflozin was analyzed. The risk factors and strategies for prevention and treatment were studied based on literature review. Results Dapagliflozin was discontinued immediately after admission, and the patient received fluid resuscitation and low dose intravenous insulin. The acidosis was corrected and ketones turned negative. Based on correlation analysis, the causal relationship between dapagliflozin and euDKA was determined as “probable”. Conclusion The use of dapagliflozin in high-altitude hypoxic environments may elevate the risk of euDKA, warranting increased clinical vigilance, prompt identification, and immediate interventions to ensure safe medication.
