Abstract: Objective To analyze one case of paroxysmal tremor associated with roxadustat capsules in order to provide data for clinical safety. Methods The clinical data of an elderly patient with stage 5 chronic kidney disease (CKD5) was studied, who developed paroxysmal tremor during roxadustat therapy for renal anemia. Literature was reviewed to find out about the incidence, potential mechanisms, and management strategies of tremor induced by roxadustat. Results The patient presented with recurrent tremor following standard-dose roxadustat administration, which was resolved completely after drug discontinuation. Based on causality assessment, the tremor was likely attributed to roxadustat capsule administration. Conclusion Clinicians should remain alert to tremor-related adverse drug reactions in elderly patients receiving roxadustat for renal anemia. Early detection and prompt interventions are critical to mitigating severe neurological complications.
