1例房颤患者服用华法林抗凝治疗的药学监护

摘要： 1例86岁男性房颤患者,入院后给予华法林抗凝治疗,期间发现患者国际标准化比值（INR）波动较大。临床药师从疾病及药物相互作用等多方面因素进行分析,同时结合药物代谢动力学和药效学特点,向临床提供合理建议,对患者进行用药指导,保障患者用药安全。

关键词: 房颤, 华法林, 抗凝, 影响因素

Abstract: A 86-year-old male patient with atrial fibrillation was given warfarin for anticoagulation therapy in hospital. The international normalized ratio(INR) of the patient fluctuated during the period of therapy. Clinical pharmacists analysed disease,drug-drug interactions and other influence factors,combined with the characters of pharmacokinetics and pharmacodynamics to provide physicians with some reasonable advices. Clinical pharmacists also provided medication education to the patient, and played an important role in the safety of drug use.
