注射用芦康沙妥珠单抗致癌症患者急性疼痛2例分析

Two cases of acute systemic pain caused by sacituzumab tirumotecan for injection in cancer patients

摘要: 目的 探讨注射用芦康沙妥珠单抗所致急性疼痛不良反应,评估其临床表现、机制及处理策略。方法 回顾性分析2例接受该药物治疗后出现急性疼痛的癌症患者,收集其基本资料、不良反应发生时间、疼痛表现及处理措施,并结合相关文献分析可能机制。结果 2例患者均在静脉滴注后4 h内出现急性疼痛,经对症治疗及停药后症状逐渐缓解。文献提示,作为人滋养层细胞表面抗原2(TROP-2)靶向抗体偶联药物,注射用芦康沙妥珠单抗可能通过炎症介质释放或“旁观者效应”等机制诱发疼痛。结论 注射用芦康沙妥珠单抗可能引发急性疼痛不良反应,临床需警惕急性重度疼痛风险,建议用药期间加强监测并及时干预。

Abstract: Objective To explore such adverse drug reactions (ADRs) as acute pain caused by sacituzumab tirumotecan for injection and to evaluate its clinical manifestations, potential mechanisms, and management strategies. Methods A retrospective analysis was conducted of two cancer patients who developed acute pain following treatment. Data on patients’ demographics, time to ADR onset, pain manifestations, and interventions was collected. Potential mechanisms were analyzed with reference to the literature. Results Both patients developed acute pain within 4 hours of intravenous infusion. Symptoms were gradually resolved after symptomatic treatment and drug discontinuation. Literature suggested that as a TROP-2-targeted antibody-drug conjugate (ADC), sacituzumab tirumotecan for injection might caused pain through such mechanisms as inflammatory mediator release or the “bystander effect”. Conclusion Sacituzumab tirumotecan for injection may cause acute pain. Clinicians should be vigilant about this risk, and close monitoring and prompt interventions during treatment are recommended.
