复方氨酚烷胺片对感冒期精神分裂症患者利培酮血药浓度的影响

Effect of Compound Paracetamol Tablets on Plasma Concentrations of Risperidone in Patients with Schizophrenia Complicated with a Cold

摘要: 目的 分析感冒和感冒期使用复方氨酚烷胺片对精神分裂症患者利培酮血药浓度的影响。方法 回顾性收集2019年6月1日至2024年6月1日在本院接受利培酮治疗的感冒患者424例,并将感冒期服用复方氨酚烷胺片患者296例纳入服药组,感冒后没有进行干预患者128例纳入未干预组,分析感冒及复方氨酚烷胺片对精神分裂症患者利培酮血药浓度的影响。结果 ①服药组,感冒期与基线期比较,高水平利培酮血药浓度比例增加(P<0.05);②2组患者感冒期较基线期,利培酮血药浓度水平均有不同程度升高(P<0.05),服药组比未干预组升高更明显(P<0.05);③感冒期,服药组青壮年(18~45岁)的利培酮C/D性别差异无统计学意义(P>0.05),中老年(46~60岁)的女性利培酮C/D大于男性,性别差异有统计学意义(P<0.05);④2组与利培酮血药浓度升高相关的不良反应发生率无显著差异(P>0.05),但2组患者高浓度水平时发生不良反应的例数占50%以上。结论 感冒可能会升高精神分裂症患者利培酮血药浓度,感冒期使用复方氨酚烷胺片可能会进一步升高血药浓度,建议必要时行血药浓度监测及合理调整药物治疗方案。

Abstract: Objective To analyze the effect of compound paracetamol tablets on plasma concentrations of risperidone in patients with schizophrenia who had a cold. Methods The clinical data of 424 cold patients who received risperidone treatment in a hospital between June 1, 2019 and June 1, 2024 was retrospectively collected, 296 of whom had taken compound paracetamol alkane tablets while they had a cold and were assigned to the medication group. The rest of the patients who received no medication for their cold were included in the non-intervention group. The effects of colds and compound paracetamol tablets on high-level plasma concentrations of risperidone in these patients were analyzed. Results ①In the medication group, plasma concentrations of risperidone increased to varying degrees compared with the baseline (P <0.05).②The plasma concentrations of risperidone in both groups increased to varying extents compared with the baseline (P <0.05), especially in the medication group (P <0.05).③There was little difference in levels of risperidone between male and female patients ages 18 to 45 in the medication group (P >0.05), but the plasma concentrations of risperidone were higher in females than in males aged 46 to 60 (P <0.05).④There was no significant difference in the incidence of adverse reactions related to risperidone between the two groups (P >0.05), but more than half of the cases of adverse reactions occurred among patients with high concentrations of risperidone. Conclusion Colds may increase the plasma concentration of risperidone in patients with schizophrenia. Using compound paracetamol tablets can make matters worse among patients with a cold. It is recommended that blood concentrations be monitored and medication plans be adjusted when necessary.
