The reasons of errors were given, and the countermeasures were suggested. ResultsThere were 455 irrational prescriptions intervened from 2012 to 2014. The rate of the irrational prescriptions was reduced from 2.4 to 0.6 (P=6.6×10-32, P<0.05). Dermatology, cardiology internal, endocrinology, otolaryngology and gastroenterology were most frequently involved in irrational prescriptions. ConclusionInterventions are effective. There were still problems in prescription writing and rational drug use, which need further improvement in order to ensure the patient’s safety. Pharmacists must play the role of professionals.
